SUBMISSION GUIDELINES

  • Accounts must have minimum of 3 years in business
  • Accounts must have 3 or more full time employees
  • Accounts must be responsive to loss prevention recommendations with management support
  • Complete ACORD Application and Healthcare Supplemental Application signed by insured or Broker, including prior carrier information, payroll and employee count per location
  • Experience Rating Worksheet (If Available)
  • Details of all losses over 50k
